Position: Physical Therapist (PT) Home Health
Location (Office Base): Ladson SC (Home Health field-based role)

Schedule:
Monday to Friday 8:00 AM 5:00 PM (No on-call)

Role Overview:
The Physical Therapist will provide patient care in a home health setting managing a designated territory. The role involves evaluating planning and delivering therapy services to help patients improve mobility strength and overall functional ability.

Key Responsibilities:

Provide hands-on patient care as per plan of care

Document all treatments and patient progress in EMR

Supervise Physical Therapy Assistants (PTAs) and Home Health Aides

Coordinate care with interdisciplinary teams families and agencies

Manage patient caseload within assigned territory

Requirements:

Active Physical Therapist license in South Carolina

Current AHA BLS Certification

Minimum 1 year of PT experience

Home Health experience is mandatory

Valid drivers license (travel required)
