Job Summary
DAY SHIFT OUTPATIENT PHYSICAL THERAPIST
Shift: Clinic is open Monday - Friday. Candidate would get 32hrs/week with the opportunity to work late days and should be flexible on scheduling.
IMPORTANT NOTE: Due to how billing is done at the Clinic Traveler would need to have provider enrollment completed prior to seeing patients. They bill on separate NPI numbers so each PT needs to be run through Provider Enrollment for each insurance company that they bill. It sometimes takes 60 days for the insurance to approve the enrollment. This is due to how they bill for their services. This would be handed directly by NCH credentialing but can take some time. Would need someone who is flexible with their start date based on enrollment.
LOCALS permitted for $72 bill rate.

Requirement description :
- Minimum 2 years current Outpatient PT experience REQUIRED
- Previous travel experience HIGHLY PREFERRED
- Work History
- Physical Therapy skills checklist REQUIREDDOCUMENT
- 1 Supervisory Signed Reference from the past year of employment REQUIRED
- Active American Heart Association BLS - REQUIREDDOCUMENT
- License verification REQUIREDDOCUMENT
- All licenses/certs must be current at time of submission
- Permanent address must be included in submission file REQUIRED
- RTO - must be included in submission
