Job Overview:The Physical Therapist (PT) is responsible for delivering prescribed physical therapy treatments using advanced modalities to help patients achieve their maximum rehabilitative potential. This role involves evaluating patient conditions developing care plans and providing hands-on therapy to improve mobility strength safety and self-care abilities. Schedule:
Monday to Friday (8:00 AM 5:00 PM)
No on-call requirements
Assigned territory-based patient visits (Home Health setting)
8-hour workday managing patient caseload
Key Responsibilities:
Provide direct hands-on patient care according to the established plan of care
Evaluate plan and implement physical therapy treatments
Document all patient care services goals and coordination accurately in EMR systems
Supervise Physical Therapy Assistants (PTAs) and Home Health Aides
Coordinate care with interdisciplinary teams families and external agencies
Ensure treatment goals and patient outcomes are achieved
Operate a vehicle to travel within assigned territory
Requirements:
Licensing/Certification:
Active Physical Therapist license in the state of South Carolina
Current BLS certification (American Heart Association Healthcare Provider)
Education:
Graduate from an accredited Physical Therapy program
Experience:
Minimum 1 year of experience as a Physical Therapist
Home Health experience is mandatory (No exceptions)
Skills & Competencies:
Strong understanding of therapeutic exercises and rehabilitation techniques
Knowledge of orthotic prosthetic and assistive devices
Excellent organizational and time management skills
Ability to manage patient caseload independently
Specialty competency (e.g. pediatrics) as applicable
Additional Information:
Valid drivers license required
Mileage reimbursement provided at IRS 2026 rate
