Synergy Care is seeking a full-time licensed Physical Therapist to join our team in Fort Worth TX. This position is located within an Independent Living facility and includes an on-site outpatient clinic providing care to residents in a supportive and collaborative environment.
Responsibilities
- Evaluate and treat patients using evidence-based physical therapy techniques
- Develop and implement individualized treatment plans
- Monitor patient progress and adjust care as needed
- Collaborate with interdisciplinary team members
- Maintain accurate and timely documentation
Qualifications
- Graduate of an accredited Physical Therapy program
- Current (or eligible for) Physical Therapist license in Texas
- Strong communication and patient care skills
- New graduates are welcome to apply
