What Youll Do:

• Provide skilled nursing care in patients homes
• Perform patient assessments and develop individualized care plans
• Administer medications and treatments as ordered
• Monitor patient progress and update physicians as needed
• Educate patients and families on disease management and care
• Complete accurate and timely EMR documentation
• Collaborate with interdisciplinary care teams

What Were Looking For:

• Licensed Registered Nurse (RN) in Maryland or eligible
• 2 years RN experience
• Home health experience is a plus not required
• Strong clinical communication and organizational skills
• Knowledge of Medicare/Medicaid guidelines preferred
• Ability to work independently and manage time effectively
