Human Touch Home Health is seeking a licensed Physical Therapist (PT) for a PRN (as-needed) home health position serving patients in Blacksburg and surrounding communities.

This role is ideal for clinicians seeking flexibility and autonomy while providing meaningful one-on-one care in the home setting.

What Youll Do:

• Provide home health physical therapy services to adult and geriatric patients
• Perform evaluations and develop individualized care plans
• Deliver rehabilitation services focused on strength balance and mobility
• Educate patients and caregivers on treatment plans
• Collaborate with physicians and interdisciplinary care teams
• Complete accurate and timely EMR documentation

What Were Looking For:

• Licensed Physical Therapist (PT) in Virginia
• Doctor of Physical Therapy (DPT) required
• Minimum of 12 years of clinical experience required
• Home health experience preferred not required
• Strong communication and time management skills
• Ability to work independently and travel between patient homes
