Job Title: Physical Therapist

Location: Ladson SC

Schedule: Monday Friday 8:00 AM 5:00 PM

Job Summary:

We are seeking a skilled Physical Therapist to provide high-quality rehabilitative care in a home health setting. The Physical Therapist will evaluate patients develop treatment plans and deliver therapeutic interventions aimed at restoring mobility strength and overall functional independence.

Responsibilities:

• Evaluate patients and develop individualized physical therapy treatment plans
• Provide hands-on therapeutic care in accordance with established plans and standards of practice
• Improve patient mobility strength safety and functional abilities
• Document all patient care progress and coordination of services accurately in the EMR
• Supervise Physical Therapy Assistants (PTAs) and Home Health Aides to ensure treatment goals are met
• Coordinate care with interdisciplinary teams families and external providers
• Utilize therapeutic exercises modalities and assistive devices to support rehabilitation
• Educate patients and caregivers on treatment plans home exercises and safety measures
• Maintain compliance with clinical safety and regulatory standards

Required Qualifications:

• Minimum 1 year of experience as a Physical Therapist
• Home Health experience required
• Current Physical Therapist license in the state of South Carolina
• Current BLS Certification (American Heart Association)
• Valid drivers license and ability to travel for patient visits

Education:

• Graduate of an accredited Physical Therapy program

Key Skills:

• Strong knowledge of rehabilitative therapy techniques and modalities
• Experience with orthotic prosthetic and assistive devices
• Excellent organizational communication and time management skills
• Ability to work independently in a home health environment
