Job Title: Physical Therapist
Location: Ladson SC
Duration: 13 Weeks
Schedule: Monday Friday 8:00 AM 5:00 PM
Employment Type: Contract
Job Summary:
We are seeking a skilled Physical Therapist to provide high-quality rehabilitative care in a home health setting. The Physical Therapist will evaluate patients develop treatment plans and deliver therapeutic interventions aimed at restoring mobility strength and overall functional independence.
Responsibilities:
- Evaluate patients and develop individualized physical therapy treatment plans
- Provide hands-on therapeutic care in accordance with established plans and standards of practice
- Improve patient mobility strength safety and functional abilities
- Document all patient care progress and coordination of services accurately in the EMR
- Supervise Physical Therapy Assistants (PTAs) and Home Health Aides to ensure treatment goals are met
- Coordinate care with interdisciplinary teams families and external providers
- Utilize therapeutic exercises modalities and assistive devices to support rehabilitation
- Educate patients and caregivers on treatment plans home exercises and safety measures
- Maintain compliance with clinical safety and regulatory standards
Required Qualifications:
- Minimum 1 year of experience as a Physical Therapist
- Home Health experience required
- Current Physical Therapist license in the state of South Carolina
- Current BLS Certification (American Heart Association)
- Valid drivers license and ability to travel for patient visits
Education:
- Graduate of an accredited Physical Therapy program
Key Skills:
- Strong knowledge of rehabilitative therapy techniques and modalities
- Experience with orthotic prosthetic and assistive devices
- Excellent organizational communication and time management skills
- Ability to work independently in a home health environment
