Hours: 32 hours per week
Schedule: Monday Sunday (Every other weekend required)
Job Summary:
Navitas Healthcare LLC is seeking a compassionate and dedicated Mental Health Counselor to deliver comprehensive behavioral health services. This role involves patient assessment treatment planning and implementation of therapeutic interventions. The ideal candidate will collaborate with a multidisciplinary team to support patient recovery and ensure the highest standards of care.
Key Responsibilities:
Conduct patient assessments and collect relevant clinical data to develop individualized treatment plans
Monitor and evaluate patient progress reporting any significant changes
Provide both individual and group therapy sessions based on patient needs
Educate patients and involve family members in care planning when appropriate
Coordinate care with healthcare providers including primary care physicians and outpatient services
Participate in multidisciplinary team meetings to ensure effective care delivery
Manage case coordination including insurance verification and service authorizations
Maintain accurate and timely documentation of clinical interventions and patient responses
Support case management efforts to ensure continuity of care and positive outcomes
Assist in supervising students and interns as applicable
Ensure compliance with organizational policies standards of care and documentation requirements
Qualifications:
Bachelors Degree (BA/BS) in Psychology Human Services or a related field (Masters preferred)
Minimum of 1 year of clinical experience in behavioral health or human services
Strong interpersonal and communication skills
Ability to work effectively with diverse patient populations
Experience with Meditech EMR preferred
Active state licensure required (LPC or LCSW)
Certifications & Requirements:
AHA BLS Certification (required or must be obtained within 90 days)
Must maintain active state licensure throughout the assignment
Ability to meet the physical and mental demands of the role
