Position Highlights

Work location: Texas Health Fort Worth 1304 Pennsylvania Ave Fort Worth TX 76104

Work environment: Palliative Care services at Texas Health Fort Worth a Magnet designated Hospital Level I Trauma Center conveniently located close to downtown Fort Worth.

Work hours: Full-time 40 hours/week Monday through Friday 8 am - 5 pm No Weekend requirement.

Texas Health Harris Methodist Hospital Fort Worth is a 833-bed Magnet-designated regional referral center that has served the residents of Tarrant County and surroundings communities since 1930. Hospital services include cardiovascular care high-risk obstetrics gynecology orthopedics and sports medicine neonatal intensive care and trauma/emergency early 2014 Texas Health Fort Worth opened a new 75000 square foot Emergency Services Center one of the largest in the region. Texas Health Fort Worth has more than 4000 employees 200 volunteers and nearly 1000 physicians on its medical staff. Texas Health Fort Worth recently received national recognition for its oncology program and was named Best Place to Have a Baby in Tarrant County. The hospital is also a designated Cycle III Chest Pain Center by the Society of Chest Pain Centers

Heres what you need:

BSN required

Current RN licensure or compact licensure recognized by the Texas Board of Nursing upon hire

3 years experience in Palliative or Hospice care required

The successful Palliative Care Program Coordinator will have the following skills:

Knowledge of palliative care and hospice services philosophy and end of life issues. Comfort in dealing with death and dying issues complex social spiritual and personal issues and the ability to counsel and support patients and families with dignity and respect regardless of their personal belief.

Comfort with advance directive counseling and end-of-life conversations. Ability to speak to palliative and hospice philosophy and practice to advocate service line to clinical and medical staff.

Ability to coordinate and collaborate with a multidisciplinary team to provide palliative and hospice care services.

Knowledge of community resources including Medicare/Medicaid financial aid county state and private social services agencies.

Must be able to work independently have demonstrated patient care planning and patient teaching skills and strong written and verbal communication skills.

Must have general computer skills and a working knowledge of electronic heath records MS Office Outlook and the Internet. Experience with Microsoft Office CareConnect and EPIC preferred

What you will do:

Provide care consistent with National Hospice and Palliative Care Organization standards.

Assess patient/family social and emotional factors in order to estimate their potential to cope with terminal illness and death.

Provide psychosocial counseling and support to the patient/family experiencing emotional social and economic conflict.

Assist patient/family and staff in utilizing community resources.

Assess patient/family financial and insurance status and assist with appropriate resources as indicated.

Participate in Daily Care Briefing to establish review and revise the plan of care for each patient including exchange of information review of problems of professional support.

Collaborate with clinical and medical staff to improve code status discussions for all admissions.

Responsible for maintenance of electronic medical records assuring accuracy completeness and compliance with regulations certification standards ethical standards.

Support instruct and educate the patient family and caregiver.

Coordinate all patient and family services.

Maintain the dignity of the dying patient.

Support the patients and familys unique spiritual and cultural beliefs.

Provide holistic family-centered care across treatment settings to improve the quality of life.

Consult and collaborate with the interdisciplinary team and others involved in the patients care.

Participates in the development and implementation of a palliative care and hospice service line.

Leadership in the planning organization communication and evaluation of all activities of the palliative care program.

Coordinates care for Palliative and Hospice patients with Care Transition Management Pastoral care Medical Staff and Clinical staff excellent patient care for patients in need.

Act as primary interface/liaison with local Palliative and Hospice providers in conjunction with Care Transition Management to advance end-of-life and palliative care.

Collection of data pertinent to the determination of quality of palliative and hospice care services both as inpatient and post-acute necessary by the facility.

Develop and lead educational efforts to advance knowledge and skills of clinical and medical staff to promote palliative and hospice

Advocate for palliative care and hospice care when appropriate lead efforts to promote palliative and hospice services as a clinical option with advanced disease in need of holistic care and complex symptom relief.

Community education for advanced directives end-of-life care and palliative care services.

Participate in the hospital ethics committee.
