Compensation & Benefits
- Salary Range: $109762 $135000 (commensurate with experience)
- Annual Incentive Bonus: Up to 17.5%
- Sign-On Bonus: $10000
- Relocation Assistance: Available on a case-by-case basis
- Schedule: Full-time Day Shift (Administrative hours)
- Work-Life Balance: No weekends required (occasional late coverage/on-call as needed)
Position Overview
A leading 407-bed Level I Trauma Center and comprehensive acute care facility is seeking an experienced Director of Case Management to provide strategic and operational leadership for its Case Management department. This high-acuity environment supports nearly 18000 annual admissions and requires strong leadership to optimize patient flow resource utilization and quality outcomes.
Reporting to the Division Vice President of Case Management you will oversee a team of approximately 26 FTEs and serve as the primary leader for care coordination discharge planning and utilization management across the facility.
Key Responsibilities
Strategic Leadership
- Lead and optimize the hospitals case management model with a focus on reducing length of stay (LOS) and improving throughput
- Develop and implement strategies to enhance patient flow and hospital capacity management
Operational Oversight
- Manage staffing structure hiring onboarding training and competency development
- Ensure departmental productivity efficiency and performance standards are consistently met
Data & Performance Improvement
- Analyze utilization and operational data to identify trends and performance gaps
- Develop and execute improvement initiatives based on data-driven insights
Cross-Functional Collaboration
- Partner closely with executive leadership including CFO CMO and CNO
- Collaborate with interdisciplinary teams and external stakeholders to ensure continuity of care
Regulatory & Compliance
- Ensure adherence to all internal policies procedures and regulatory requirements
- Maintain compliance in discharge planning and utilization management standards
Qualifications
Education
- Bachelors degree required
- Masters degree in Nursing (MSN) Healthcare Administration (MHA) or MBA preferred
Licensure
- Active RN license LCSW or LMSW required
Experience
- Minimum 3 years of recent acute care hospital case management experience
- At least 2 years of leadership experience in case management or care coordination

- Strong executive presence and leadership capability
- Advanced analytical and data interpretation skills
- Proven experience in change management and process improvement
- Ability to navigate complex clinical and operational environments with professionalism and diplomacy
