What Youll Do:
- Provide home health physical therapy services to adult and geriatric patients
- Perform evaluations and develop individualized care plans
- Deliver rehabilitation services focused on strength balance and mobility
- Educate patients and caregivers on treatment plans
- Collaborate with physicians and interdisciplinary teams
- Complete accurate and timely EMR documentation
What Were Looking For:
- Licensed Physical Therapist (PT) in DC Maryland or Virginia (or eligible)
- Doctor of Physical Therapy (DPT) required
- New grads welcome mentorship available
- Home health experience preferred not required
- Strong communication and time management skills
- Ability to work independently and manage your schedule
