Empath Home Health a member of Empath Health is seeking a caring full-time Home Health LPN to work with us to deliver individualized care in north Pinellas & West Pasco counties (Hudson/New Port Richey area down to Clearwater/Largo area).

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home along with personal care support that assists with daily needs as part of a seamless continuum of care.

Locations: Home health visits in west Pasco county (including Hudson New Port Richey Trinity and Holiday) and north Pinellas county (including Clearwater Largo Palm Harbor Safety Harbor Dunedin and Tarpon Springs).

Schedule: M-F 40 hours; weekend and holiday on-call rotations required.

What youll Do

• Provides nursing services in compliance with agency policy and procedure state and federal regulations and the patients plan of care under the direction and supervision of a Registered Nurse.
• Observes patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
• Assists the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
• Performs patient and family instruction on diet medication and treatment.
• Collaborate with the interdisciplinary team to ensure seamless holistic care

What Youll Need

• Active Florida LPN license
• Applicants must be a graduate of an approved nursing program
• Requires 1-2 years of LPN experience
• Home health care experience preferred.
• Strongly prefers experience using Homecare Homebase (HCHB) EMR
• Intravenous (IV) Certification strongly preferred.
