We are seeking a compassionate and experienced Licensed Social Worker to provide high-quality client-centered support through a fully remote setting. This role involves working with diverse populations helping individuals and families navigate challenges related to mental health social services and overall well-being.
The ideal candidate is empathetic detail-oriented and comfortable delivering services virtually while maintaining strong ethical and professional standards.
Key Responsibilities
Provide virtual counseling case management and psychosocial support to clients
Conduct comprehensive client assessments and develop individualized care plans
Connect clients with appropriate community resources and support services
Maintain accurate and timely documentation in compliance with regulatory standards
Collaborate with interdisciplinary teams healthcare providers and external agencies
Monitor client progress and adjust care plans as needed
Ensure compliance with all federal state (DC) and organizational guidelines
Participate in team meetings supervision and ongoing training
Requirements
Required Qualifications
Active Licensed Social Worker (LSW LCSW or equivalent) in Washington DC (required)
Masters degree in Social Work (MSW) from an accredited program
Minimum 2 years of professional social work experience (clinical or non-clinical)
Experience with telehealth or remote service delivery preferred
Strong knowledge of DC social service systems and resources
Excellent communication organizational and interpersonal skills
Ability to work independently in a remote environment
Preferred Qualifications
Clinical experience (LCSW preferred for therapy-focused roles)
Experience working with diverse or underserved populations
Familiarity with electronic health records (EHR) systems
Bilingual skills are a plus
Required Skills:
Responsibilities: - Provide direct patient care under the supervision of a Physical Therapy Occupational Therapy & Speech language Therapy. - Collaborate with the healthcare team to develop and implement care plans. - Provide emotional support to patients and their families. - Document patient information accurately and timely.
Required Education:
. College Degree or equivalent. At least 2 years experience in a Staffing role performing similar work duties. Experience with Microsoft Office Suite
