Position Overview

The LPN will be responsible for patient assessments medication administration and accurate documentation while supporting overall patient care and treatment plans.
Key Responsibilities
- Perform patient assessments and monitor health conditions
- Administer medications in accordance with physician orders

Required Qualifications
- Active Licensed Practical Nurse (LPN) license in the state of Illinois
- Diploma or degree from an accredited nursing program
- Strong clinical organizational and communication skills
