Key Responsibilities
Perform patient assessments and monitor health conditions
Administer medications in accordance with physician orders
Maintain accurate and timely patient records
Assist in the development and implementation of patient care plans
Support daily patient care activities to ensure comfort and safety
Collaborate with the healthcare team to deliver effective care
Required Qualifications
Active Licensed Practical Nurse (LPN) license in the state of Illinois
Diploma or degree from an accredited nursing program
Strong clinical organizational and communication skills
