This opportunity is located at our Family Medicine Care Zionsville office at 55 Brendon Way Suite 100 Zionsville IN 46077.

Community Health Network has an excellent opportunity for a family medicine trained Advanced Practice Provider to work at our outpatient Family Medicine Care Zionsville practice.

• A comprehensive and integrated electronic medical record system (EPIC)

Experience

• Family Medicine experience as a Nurse Practitioner is recommended

• Must successfully complete a nationally recognized Advanced Practice Nurse certification examination or have current national certification; must maintain certification on an ongoing basis.

• Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing; must maintain eligibility for prescriptive authority on an ongoing basis.
