Position Overview
Navitas Healthcare LLC is seeking a detail-oriented and patient-focused Pharmacist I (Inpatient) to join a dynamic healthcare team on a contract basis. This role is responsible for ensuring the safe accurate and effective dispensing of medications while providing essential pharmacological guidance to patients and the multidisciplinary care team.
Key Responsibilities
- Accurately dispense medications in a timely manner in accordance with prescriptions and established policies
- Review and resolve inquiries related to formulary and non-formulary medications
- Provide evidence-based dosing and therapeutic recommendations based on patient-specific factors (e.g. age pharmacokinetics comorbidities)
- Monitor patient drug therapies to ensure safety efficacy and compliance
- Educate patients on:
- Proper medication usage and dosage
- Timing and adherence
- Potential side effects
- Drug interactions
- Collaborate with physicians nurses and other healthcare professionals to optimize patient outcomes
- Adhere to all departmental policies procedures and regulatory standards
Required Qualifications
- Graduate of an ACPE-accredited College of Pharmacy
- Active Pharmacist license in the state of practice
- 02 years of experience as a licensed Pharmacist (inpatient experience preferred)
- Strong knowledge of pharmacology drug interactions and clinical practices
- Excellent communication and patient education skills
