Registered Nurse RN Home Health 1:1 Patient Care

What you will do

Manage and coordinate your own patient caseload
Complete patient assessments and reassessments
Develop and update individualized care plans
Perform start of care and follow up visits
Communicate changes in patient condition with physicians and the care team
Collaborate with an interdisciplinary team to improve patient outcomes

What you need

Active RN license
1 year of clinical nursing experience
Ability to travel within the service area
Valid drivers license and reliable transportation
Home Health or Hospice experience preferred
HomeCare HomeBase experience preferred
