POSITION SUMMARY

Our Wellness Nurse (LPN) plays a vital role in ensuring the health and well-being of our collaboration with the Health and Wellness Director this role provides high-quality care through health assessments medication management and coordination with residents families and the care team. Responsibilities include monitoring resident health addressing changes in condition staff training and supervision and promoting a safe and supportive environment that aligns with our mission to provide exceptional care.


*This role is PRN on the day shift; weekend availbility a plus.


KEY RESPONSIBILITIES

  • Perform resident assessments and reassessments according to policies monitoring any changes in condition.
  • Communicate with the Health and Wellness Director about resident needs to ensure consistent quality of services.
  • When necessary participate in hands-on care medication administration and specific nursing tasks.
  • Schedule and assign caregiving and med tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances.
  • Supervise train and support care team members promoting teamwork and adherence to care standards.
  • Assist in the transcription of physicians orders and the review of new physicians orders.
  • Maintain accurate and up-to-date resident records including care plans incident reports and medical documentation.
  • Coordinate care planning with home health agencies on-site working within community and company policies.
  • Address residents special needs requests and complaints.
  • Coordinate admissions with the sales team including supervising move-ins.
  • Understand the communitys care regulations along with state-specific regulations.
  • Facilitate communication between residents families and healthcare providers to ensure seamless care delivery.
Requirements

CANDIDATE QUALIFICATIONS

Education and Certifications:

  • A valid incumbent nursing license (LPN/LVN) is required.
  • High School Diploma or equivalent.
  • An associate degree or higher in nursing is preferred or a combination of education and experience. Additional coursework in management is an asset.
  • Current First Aid and CPR license or ability to obtain.

Experience Competencies and Skills:

  • At least four (4) years working in geriatric nursing including experience with medication administration delivery systems and pharmacies.
  • At least one (1) year of relevant supervisory experience including staff scheduling and management is preferred.
  • Experience with memory care is a plus.
  • Excellent attention to detail and high integrity.
  • Excellent communication skills and teamwork mindset.
  • Strong leadership and team management skills.
  • Outstanding organizational skills.
  • Positive attitude empathy patience and commitment to treating our residents with dignity and respect.
  • Ability and willingness to participate in on-call rotation.
  • Must meet all mandatory health requirements according to state regulations
