Job Summary
Minimum Qualifications
Education:
- Bachelors degree (e.g. BSW with one-year practicum working directly with individuals with mental disabilities).
- Associates degree in a health or human services field also acceptable.
- High school diploma/GED acceptable with additional experience as noted below.
Experience:
- 2 years of experience providing case management or housing services (with a Bachelors degree); OR
- 3 years of direct care experience (with an Associates degree); OR
- 4 years of direct experience providing services to individuals with mental disabilities or to homeless individuals (with HS diploma/GED).
- Veteran preferred for Veterans Shelter positions.
Certifications/Licenses:
CPR and First Aid required (obtain/maintain); valid Drivers License.
Work Eligibility: Must be authorized to work in the U.S.

About the Role
The Housing CoordinatorEntitlements Specialist supports residents in attaining permanent housing by assessing needs identifying housing opportunities and maintaining entitlement records to ensure financial and housing stability.
Primary Responsibilities
Develop and maintain relationships with housing providers landlords and the Veterans Administration.
Create and maintain community resource guides for all five boroughs.
Identify and secure permanent housing options.
Collaborate with case management staff to understand resident needs.
Serve as liaison with landlords regarding leases move-ins rent and utilities.
Document all housing search and placement efforts.
Maintain accurate entitlement case records and monthly entitlement account logs.
Track and process entitlement funds including direct deposits.
Attend mandatory meetings supervisions and trainings.
Ensure compliance with laws regulations and agency policies.
Perform other related duties as assigned.
