Job Summary

As a Case Manager for Ambulatory care you will engage in a leadership opportunity where you will influence and innovate ambulatory rehabilitation services while keeping up with the ever changing health care platform. Here youll be empowered with ownership of waitlist oversight applying your judgment to balance access urgency and capacity. Beyond individual patient planning you will help drive the evolution of the program bringing forward insights that influence care pathways service delivery and how the team responds to changing demands. You will support smooth transitions from inpatient to outpatient care navigating referral flow in a model where funding and access require thoughtful coordination across both settings. About this team:
The Christine Sinclair Ambulatory Care Centre is a robust and dynamic rehabilitation Centre located on the Ground floor at Hennick Bridgepoint Hospital. Our team supports over 15000 patients annually and provides specialized services in various programs. These include: Day Treatment MD clinics Seating Services Augmentative and Alternate Communication (AAC) and Hydrotherapy. Within our day treatment centre we focus on orthopaedics and neurological/stroke rehabilitation and have now ventured into providing rehabilitation services to patients who have medically complex needs. Our facilities showcase a modern environment with expansive windows that provide a therapeutic space for patients to participate and engage in their rehabilitative journey. Our goal is short term rehabilitation with a drive to attain functional goals. Our team is committed to continuity of care and provide supportive resources and comprehensive discharge teaching to ensure all patients are empowered on their journey to health and recovery.

In this role you will:

  • Provide leadership and oversee the coordination of care and services for patients within the Day Treatment and Ambulatory Care programs
  • Collaborate with and manage the interdisciplinary team to ensure comprehensive patient-centered care from pre-admission through community follow-up
  • Manage the referral system for the Ambulatory Care Program and clinics including reviewing referrals and communicating decisions to internal and external partners
  • Participate in and contribute to Quality Improvement initiatives projects and committees to enhance program effectiveness and patient outcomes
  • Engage and collaborate with key stakeholders to ensure accuracy and integrity of Program Performance Metrics
  • Chair and manage program weekly rounds
  • Engage in scheduling and staffing initiatives to ensure operational efficiency
  • Work in partnership with the Clinical Managers to achieve program goals targets and operational objectives
  • Liaise with external stakeholders to ensure alignment with assigned clinical pathways and regulatory guidelines
  • Ensure patient-centered assessment care planning implementation ongoing evaluation and coordination of services
  • Respond to queries related to programs and clinics
  • Perform other duties consistent with the job classification as required

Job Requirements:
Mandatory
  • Baccalaureate degree in a health-related field from an accredited educational institution
  • 5 yearsof previous clinical experience
  • Currently registered/licensed and in good standing with a relevant professional College in Ontario (College of Nurses of Ontario College of Audiologists and Speech-Language Pathologists of Ontario College of Occupational Therapists of Ontario College of Physiotherapists of Ontario College of Social Workers and Social Service Workers of Ontario)
Preferred
  • Previous experience in the role of Case Manager or Patient Navigator

  • Superior communication skills (oral and written)
  • Demonstrated leadership and management skills

Salary:Minimum - $47.994/hour; Maximum - $59.956/hour
