Job Summary
Job Title: LVN/LPN Long Term Care (LTC) Auto Offer
Location: Rochester NY
Assignment Type: Travel Contract
Duration: 8 Weeks
Schedule: Rotating Shifts 5x8 Hours
Shift Timing:
- 3:00 PM 11:30 PM
- 11:00 PM 7:30 AM
Weekend Requirement: Every Other Weekend
Job Summary
We are seeking an experienced LVN/LPN Long Term Care (LTC) to provide high-quality nursing care in a skilled nursing facility setting. This role involves supporting patient care administering medications and assisting with daily clinical operations while ensuring patient safety and comfort.
Responsibilities
- Provide direct patient care in a long-term care or skilled nursing facility
- Administer medications and treatments as prescribed
- Monitor patient conditions and report changes to the RN or physician
- Assist with care planning and implementation
- Perform basic nursing procedures and support daily living activities
- Maintain accurate documentation in the EMR system
- Supervise and support nursing assistants as needed (charge role responsibilities)
- Ensure compliance with facility policies safety and infection control standards
Required Qualifications
- Active New York LPN/LVN License
- Minimum 1 year of LPN experience in a Long-Term Care (LTC) setting
- Previous Charge Nurse experience (required)
- BLS Certification
- Experience in Skilled Nursing Facility (SNF) or Nursing Home
- Epic charting experience (required)
Work Environment
- Skilled nursing or long-term care facility
- Fast-paced setting with a focus on patient-centered care
- Rotating shifts including evenings nights and weekends
