Job Summary

Shift Details:
- Shift: Evening
- Hours: 2:30 PM 10:30 PM
- Weekend Requirement: Every other weekend
- Guaranteed Hours: 37.5 hours per week
Position Overview:
We are seeking a compassionate and reliable LPN / LVN to provide quality patient care in a long-term care setting. The ideal candidate will have experience working with diverse patient populations and be comfortable floating between units within the facility.
Patient Population:
- Multiple Sclerosis (MS)
- Orthopedic patients
- Dementia and memory care patients
Key Responsibilities:
- Provide direct patient care under the supervision of an RN or physician
- Administer medications and treatments as prescribed
- Monitor patient conditions and report changes
- Assist with daily living activities and patient support
- Maintain accurate documentation using EMR systems
- Ensure compliance with infection control and safety standards
Requirements:
- Minimum 1 year of LPN/LVN experience required
- Active New York LPN/LVN license required
- BLS certification required
- Experience in long-term care preferred
- Familiarity with PointClick Care EMR preferred
