How Youll Make an Impact

Assess & Plan
- Complete in-home RN assessments; craft individualized Care Plans with client/family approval.
Coach & Supervise
- Orient caregivers to each Care Plan perform field competency checks and ensure ongoing compliance.
Monitor & Document
- Track vitals condition changes and interventions in our HIPAA-secure EMR; escalate concerns promptly.
Collaborate & Advocate
- Coordinate with physicians therapists and schedulers to keep care seamless and client-centered.
Quality & Compliance
- Maintain audit-ready client files collect initial service payments and uphold state home-care regulations.

What Youll Bring

Active Registered Nurse (RN) license in Pennsylvania
Bachelors in Nursing or related human-services field OR 2 years geriatric/home-care experience
Solid clinical assessment skills and a passion for problem-solving
CPR/First Aid certification and a valid drivers license
Warm professional communication that builds trust with families and field staff
