Overview

Starting Rate: $46.50/hour

The Residential Programs Nurse provides direct nursing oversight and management of medical and care components in accordance with Advocates and funder requirements. The Residential Program Nurse provides training supervision and oversight for both residents and staff in all aspects of medical and medication management. They will collaborate with the Nursing Director Residential Program Directors and nursing team in the implementation and follow-through of individualized service plans and documentation in accordance with the mission and regulations of the program.

Responsibilities

  • Take a lead role in assessing potential program participants at their residential site; identify the program participants needs and functional abilities.
  • Coordinate assessments for new durable medical equipment needs. Provide oversight and ensure ongoing equipment checks and regularly scheduled maintenance.
  • Participate initially and at least annually in a team assessment with the applicant family member/guardian and Residential Program Director to determine whether the applicant can manage their personal care services.
  • Complete the annual assessments related to the program participants support needs.
  • Ensure Physicians Authorization for each GAFC resident twice annually per Medicaid site policies (if applicable).
  • Prior to an applicants admission to the program complete a Medication Assessment and develop an Emergency Sheet Medication List nursing care plan guidelines protocols individualized ADL sheet and coordinate with the Residential Program Director to complete a risk plan.
  • Develop an individualized Emergency Contact Sheet which includes family members guardian physicians (PCP and specialists) clinic/hospital contact information Social Security Number MassHealth ID number date of birth gender guardianship status health care proxy marital status code status and program start date for each new resident prior to their start at the program.
  • Assist residents with all aspects of medical management including physicals referrals prescriptions appointments and treatments.
  • Coordinate with residents family members physicians other medical providers and site team regarding all medical care and changes in status treatment or medication.
  • Implement oversee and supervise medication management and best practices in accordance with agency policy relating to MAP in a manner consistent with funder regulations.
  • Provide supervision and oversight in Supported Living Program (SLP) programs and the nursing team in coordination with the Nursing Director.
  • Provide and document person-centered training to direct care staff as part of the nursing team. Develop and implement trainings as needed.
  • Promote and maintain a safe environment.
  • Complete all documentation in a timely manner to ensure the availability of accurate information in accordance with established standards.
  • Monitor and evaluate resident care and generate ideas for quality improvement.
  • Remain available on call 24 hours per day 7 days per week. Schedule and notify off days with the Nursing Director to ensure on-call coverage.
  • Actively participate in staff meetings trainings service planning meetings care coordination meetings resident meetings and family meetings.
  • Attend trainings as assigned; maintain necessary certifications (CPR MA Registration in Nursing).
  • Ensure that the individuals we support are treated with dignity and respect in accordance with the Brain Injury Services Divisions Human Rights Policy.

Qualifications

  • Current license to practice in Massachusetts as a Registered Nurse required. Current certification in CPR/Basic Cardiac Life Support required. Graduate of an accredited school of nursing with at least three (3) years of clinical experience required. CRRN preferred. Experience in management supervision and possession of a bachelors degree or equivalent preferred. Experience working with individuals with brain injuries in a community setting preferred.
  • Strong preference for candidates who demonstrate understanding of and competence in serving culturally diverse populations.
  • Ability to correlate clinical data with residents medical and nursing care plans.
  • Ability to communicate effectively both verbally and in writing and demonstrate good judgment.
  • Ability to set priorities and handle multiple simultaneous tasks organizing prioritizing planning sequencing and completing tasks independently.
  • Basic computer knowledge required.
  • Ability to understand speak read and write fluent English.
  • Must hold a valid drivers license. Must have access to an operational and insured vehicle and be willing to use it to transport individuals.
  • Ability to operate agency vehicles.
