Job Summary
We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care in a long-term care (LTC) setting within a skilled nursing facility. The ideal candidate will deliver compassionate care support residents daily health needs and collaborate with the healthcare team to ensure optimal outcomes.
Key Responsibilities
- Provide direct nursing care to residents in a long-term care environment.
- Monitor patient conditions and report changes to the healthcare team.
- Administer medications and treatments as prescribed.
- Assist with activities of daily living (ADLs) as needed.
- Maintain accurate and timely patient documentation.
- Support patient safety comfort and overall well-being.
- Collaborate with Registered Nurses physicians and interdisciplinary teams.
- Adhere to established policies procedures and safety standards.
Required Qualifications
- Active LPN license (state-specific as required).
- Experience in Long-Term Care (LTC) or skilled nursing preferred.
- Strong communication and patient care skills.
- Ability to work effectively in a team-oriented environment.
