About TDC:
Tiffin Developmental Center is a residential facility located in Northwest Ohio. We support youth aged 1017 and adults with developmental disabilities. Our large rural campus provides opportunities for residents to be active and engaged as part of the community. Direct care staff and nurses provide 24/7 support to individuals who call TDC home. We focus on stabilization and support so individuals can integrate into environments that best meet their personal wants and needs.

Key Responsibilities

  • Utilizes trauma-informed care principles in all interactions with residents families and staff. Educates residents and families on trauma-informed care practices self-advocacy and client rights.

  • Facilitates communication between interdisciplinary team members residents and families to support coordinated service delivery. Reviews and obtains necessary consents for services and treatment ensuring informed decision-making and compliance with regulatory standards.
  • Maintains necessary data and documentation to satisfy requirements of federal state and local agencies. Prepares data requiring social histories admission and discharge summaries court documents and completes forms required by law or departmental policy.
  • Assists with admission and discharge processes to ensure smooth transitions continuity of care and support for integration into the least restrictive environment.

  • Ensures all documentation meets Medicaid CMS DODD and ICF regulatory standards.
  • Demonstrates competency in behavior supports and client rights ensuring services align with person-centered and rights-based practices.
  • Provides psychosocial support during incidents collaborates in investigative processes as appropriate and ensures resident advocacy during Major Unusual Incidents (MUIs). Provides routine on-the-floor support to staff and residents to ensure effective implementation of behavioral and psychosocial interventions.
  • Attends meetings (e.g. trainings OhioISPs special teams) and court hearings; establishes and maintains working relationships with other facilities agencies and support services (e.g. Social Security ODJFS county boards courts and mental health providers) to coordinate referrals and follow-up services.
  • Facilitates Resident Council and self advocacy groups promoting resident engagement empowerment and informed decision-making and provides assistance to residents in exercising their right to vote.
  • Facilitates training for new hires and conducts or supports annual staff trainings related to trauma-informed care behavior supports client rights and interdisciplinary collaboration.
  • Completes Level of Care assessments and maintains accurate clinical documentation in accordance with federal state and departmental regulations.

Requires current license as social worker (i.e. LSW) as issued by State of Ohio Counselor Social Worker & Marriage & Family Therapist Board in accordance with ORC 4757.28. Pursuant to ORC 4757.41 (A)(5) any person engaging in social work or professional counseling as a civil service employee as defined in section 124.01 of the Revised Code who has at least two years of service as of July 10 2014 is exempt from current licensure as social worker (i.e. LSW) requirement.
