Job Summary
Make a real impact in behavioral health primary care and substance use (MAT) at Konza Prairie Community Health Center. Were hiring a compassionate LPN to support providers coordinate care and improve patient outcomes in a fast-paced team-based clinic.
Key Responsibilities
- Patient intake rooming vital signs and assisting with exams/treatments
- Nursing triage (phone calls symptom assessment urgent vs. non-urgent)
- Administer medications injections and support treatment plans
- Coordinate referrals prior authorizations diagnostic testing and follow-ups
- Support behavioral health and substance use disorder (MAT) patients
- Monitor medication adherence compliance and relapse prevention needs
- Manage prescriptions and refills with pharmacies
- Document accurately in EHR/EMR (eClinicalWorks)
- Provide patient education on medications and care plans
- Active unencumbered Kansas LPN license
- BLS (Basic Life Support) certification
- Vaccinations per ACIP healthcare guidelines
Preferred Qualifications
- Experience in behavioral health mental health substance use disorder MAT or primary care
- Experience with triage care coordination or case management
- Bilingual (English/Spanish)
