Job Summary

Key Responsibilities
- Provide direct patient care including assessments treatments and medication administration
- Conduct diagnostic testing and interpret results to inform patient care
- Coordinate follow-up care and communicate effectively with patients and care teams
- Collaborate closely with healthcare providers and multidisciplinary teams to ensure continuity of care
- Educate patients on health management and preventive care strategies
- Maintain accurate and timely documentation of patient interactions and treatments
- Current unencumbered Kansas LPN or RN license (or temporary permit) required
- Current vaccination record meeting ACIP Healthcare Worker recommendations required
- One year of primary care experience preferred
- FQHC experience preferred
- Bilingual (Spanish) preferred
