Short Description:
The Transition Specialist for Indianas Department of Correction: Division of Youth Services provides oversight to all transition-related practices in the States juvenile correctional facilities.
Complete Description:
Job Responsibilities may include but not limited to:
Facilitate the implementation of a life skills curriculum titled Next Steps.
Utilize the Transition Coordinator Handbook in an effort to standardize the transition practices.
Evaluate the five Transition Consultations to be completed by every student under the guidance of an assigned teacher.
Participate on a weekly basis in Administrative Release Committees (ARC) in each facility to confirm and reinforce the students reentry plans.
Do the Follow Up Phone Calls to released youth at 1 3 6 9 and 12 months.
The Transition Coordinators will represent the Indiana Department of Correction- Division of Youth Services at many conferences and meetings.
The Transition Coordinators will assist in the development of a partnership with Indianas Department of Workforce Development HIRE and Indiana Universitys HOPE Mentoring.
The Transition Coordinators will assist in the development of the individualized Education Learning Plan for each youth upon entry at the facility.
Work with state county and private agencies to provide re-entry servicers for youth transitioning back into the community.
Assist students with obtaining social security cards state identification drivers license birth certificates and additional documentation.
Assist students with transitioning back to public schools
Assist students with employment apprenticeship programs and military service information
