Position Title: Primary Care Physician multiple positions
Position Description: Independently provides care for patients with acute and chronic illnesses in older adult patients. Manages patient care and outcomes and appropriately seeks consultation from specialists while staying involved in and responsible for the detailed care of the patient. Engages with hospitalist whenever their patients are in the hospital (regardless of whether the hospitalist works for ChenMed). Responsible for assessment diagnosis treatment management education health promotion and care coordination and documentation for patients with acute and complex chronic health needs. For patients unable to travel to the officein hospital SNF LTC or homebound - engages with transitional care team and others including case managers acute and transitional-care physicians and other resources available. Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly and supports Center Manager/Center General Manager if the center clinical leader and/or market clinical leader is unavailable based on guidance from Market Chief Medical Officer.
May work at other Dedicated Senior Medical Center sites in Jacksonville FL.
Requirements: Medical Degree or foreign equivalent Board Certified/Board Eligible (BC/BE) in Family Medicine or Internal Medicine. Must possess a Florida State Medical license and Drug Enforcement Administration (DEA) license.
Offer Salary: $239200/year - $315915/year
Location of Position: 6841 Blanding Blvd Jacksonville FL 32244. May work at other Dedicated Senior Medical Center sites in Jacksonville FL.

PAY RANGE:
$221141 - $315915 SalaryThe posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience education geographic location and other relevant factors. This position may also be eligible for a bonuses or commissions.
