Pharmacist 1 Extended CareVeterans Centre Pharmacy Regular Full-Time
Job Summary
We currently have one regular full-time opportunity for a Pharmacist 1 in the Veterans Centre at our Bayview campus. Patient care responsibilities include the residents and community medicine patients in our Veterans program inpatients in our Hurvitz Brain Sciences Program and inpatients at the Reactivation Care Centre in our Integrated Community program. Expected start date is as soon as available.
The Veterans Centre pharmacist is a key member of the interprofessional healthcare team responsible for the provision of medication therapy management. The responsibilities of the pharmacist are broadly divided into three areas:
1. Direct patient care
Conduct medication reconciliation at transitions of care
Provide comprehensive pharmaceutical care and monitoring
Complete clinical documentation for all clinical activities
Participate in interprofessional team rounds and huddles
Complete medication reviews
Provide pharmacist managed warfarin therapy
Provide back-up coverage on a variety of patient care units
2. Drug distribution
Review verify and enter medication orders into the pharmacy information system (WORx) and Veterans Centre CPOE system (SunnyCare)
Process and check medication passes
Contact prescribers to clarify and resolve drug therapy concerns
Supervise medication distribution activities by pharmacy technicians
Respond to drug information requests
3. Educational and professional responsibilities
Provide education on medications to physicians nurses other health professionals patients and families
Support experiential education as a preceptor with the Leslie Dan Faculty of Pharmacy University of Toronto or with the School of Pharmacy University of Waterloo
Participate in pharmacy case presentations education rounds journal clubs
Develop and review policies related to pharmacy practice and operations
Enhance safety by completing and following up on patient safety reports
Engage in continuous quality improvement and practice-based research activities
Participate in hospital and departmental committees
This position requires the incumbent to work closely with other professionals to support excellent patient care proactive management of patient medications to ensure optimum patient outcomes and the ability to work well in an environment with many interprofessional team members at various stages of education.
Qualifications/Skills:
Recent prior work experience in the Extended Care or Acute Care Pharmacy Sunnybrook Health Sciences Centre preferred
Current registration as a Part A pharmacist by the Ontario College of Pharmacists
Previous hospital pharmacy experience and/or hospital pharmacy residency preferred
Board-Certified Geriatric Pharmacist (BCGP) or Board-Certified Psychiatric Pharmacist (BCPP) credential preferred (established or currently in pursuit of)
Strong clinical knowledge and application of geriatric psychiatric and palliative care principles
Able to work well in a fast-paced work environment
Demonstrated high degree of accuracy and attention to detail
Self-motivated and able to take initiative
Demonstrated ability to work well in a multidisciplinary team environment
Excellent interpersonal and communication skills
Well organized self-directed able to set priorities and manage time effectively
