The Impact Youll Make:
As a Primary Care Physician you will guide the medical care of PACE participants through comprehensive assessments individualized care planning and ongoing clinical leadership within the Interdisciplinary Team.
Key Responsibilities
In this role you will:
- Perform initial and ongoing comprehensive health assessments
- Lead clinical decision-making within the interdisciplinary care team
- Develop and oversee personalized care plans aligned with participant goals
- Partner with families to support informed compassionate care decisions
- Manage chronic conditions with a proactive preventative approach
- Collaborate with specialists and oversee care transitions including skilled nursing stays
- Work closely with the Medical Director and Advanced Practice Providers to ensure coordinated high-quality care

Skills Knowledge & Expertise
Required:
- MD or DO from an accredited program
- Board Certification in Internal Medicine or Family Medicine
- Active unrestricted Michigan medical license
- CPR certification (or willingness to obtain)
Preferred:
- Geriatrics and/or palliative care experience or certification
- Experience caring for medically complex or frail older adults
