Responsibilities
- Manage and supervise the social services team providing guidance and professional development.
- Develop and implement individualized care plans to address residents social and emotional needs.
- Coordinate with healthcare providers families and community resources to support resident care.
- Maintain compliance with all regulatory requirements and documentation standards.
- Conduct assessments and provide counseling or referrals as necessary.
- Lead initiatives to improve resident satisfaction and social engagement opportunities.
- Participate in interdisciplinary team meetings and contribute to care planning.
Requirements
- Bachelors or Masters degree in Social Work or related field.
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred.
- Minimum of 3-5 years experience in social services preferably in long-term care or healthcare settings.
- Strong leadership and communication skills.
- Ability to work collaboratively with multidisciplinary teams and families.
- Knowledge of local community resources and regulatory standards.
