Job Summary
The Director of Social Services at Willow Tree Care Center will lead and oversee the social services department ensuring the delivery of compassionate and quality care to our residents. This role involves coordinating social work activities developing supportive programs and collaborating with healthcare professionals to meet the emotional social and psychological needs of our community. The ideal candidate will bring strong leadership skills and a commitment to enhancing the wellbeing of our residents.
Responsibilities
- Manage and supervise the social services team providing guidance and professional development.
- Develop and implement individualized care plans to address residents social and emotional needs.
- Coordinate with healthcare providers families and community resources to support resident care.
- Maintain compliance with all regulatory requirements and documentation standards.
- Conduct assessments and provide counseling or referrals as necessary.
- Lead initiatives to improve resident satisfaction and social engagement opportunities.
- Participate in interdisciplinary team meetings and contribute to care planning.
Requirements
- Bachelors or Masters degree in Social Work or related field.
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred.
- Minimum of 3-5 years experience in social services preferably in long-term care or healthcare settings.
- Strong leadership and communication skills.
- Ability to work collaboratively with multidisciplinary teams and families.
- Knowledge of local community resources and regulatory standards.
