As part of the team supporting this new program at the 16x Burnside Recovery Center you will help provide 3.7 Level of Care stabilization offering individual and group therapy integrated mental health and medical services and community-based management and care coordination. We ensure expedited coordinated access to primary care mental health outpatient substance use disorder services and housing with a commitment to inclusive and culturally specific service delivery for individuals with substance use disorders as a primary diagnosis and co-occurring mental health conditions. The Psychiatric Mental Health Nurse Practitioner (PMHNP) works collaboratively as a member of a multi-disciplinary team providing SUDS and Mental Health services.


Salary Ranges from: $164000-$213000/annually *Range is professional experience and licensure/certifications

Schedule: Monday-Friday 8:30am-5:00pm

Location: 16th & BurnsideRecovery Center (1616 E Burnside St. Portland OR 97214)

Minimum Qualifications:

  • Bachelors degree in nursing and Masters degree in Psychiatric/Mental Health Nursing from State Board of Nursing-approved schools of nursing.
  • Current license to practice nursing in Oregon and certified as a PMHNP by the Oregon State Board of Nursing and National certification.
  • Must have full prescriptive powers in Oregon with a DEA number.
  • Must meet CCC privileging requirements as required by FTCA.
  • Current BLS certification required prior to start.
  • Bilingual in Spanish preferred.
  • Must have full prescriptive powers in Oregon with a DEA number.
  • Must meet applicable training requirements for Buprenorphine prescribing.
  • Must generally have the ability to walk bend stoop kneel squat twist reach pull and lift heavy objects and climb stairs with reasonable or no accommodation.
  • Must pass a pre-employment drug screen TB test and background check. This includes clearance by the DHS Background Check Unit.
  • The OAR prohibits SUDS program staff from having within the previous 3 years a DUI conviction or diversion a felony conviction or a conviction under the Uniform Controlled Substances Act.
  • Must be able to work within an integrated multidisciplinary setting.

Essential duties and responsibilities:

  • Work as a member of a multi-disciplinary team to deliver comprehensive client centered and trauma informed integrated services.
  • Actively participate in the integration of services and identify new opportunities to improve collaboration amongst team.
  • Proactively identify opportunities for improvement within workflows and logistics.
  • Build understanding of and appreciation for diversity among clients and support their connection to culturally responsive and culturally specific services as requested.
  • Provide direct patient care including but not limited to: psychiatric evaluation consultation psychopharmacological and psychotherapeutic interventions and monitoring and addressing adverse effects.
  • In collaboration with the multi-disciplinary team to support milieu staff with milieu management challenges.
  • Facilitate therapy groups
  • Prescribe and manage the opioid agonist medication buprenorphine as well as other medication supported recovery medications such as Vivitrol Naltrexone and Acamprosate. As well as educate clients on medication management
  • Document plan for medical and psychiatric treatment and on-going treatment in the outpatient setting.
  • Provide crisis intervention suicide assessment and intervention.
  • Conduct care of psychiatric/mental health emergencies and initiate arrangements for continuing care or referral to appropriate facility.
  • Participate in evaluating clients for risk of harm and initiate Directors Custody transport holds when appropriate.
  • Provide brief psychotherapy interventions including motivational interviewing.
  • Provide patient and family education.
  • Provide clinical consultation and training to staff
  • Participate in clinical staff meetings and case conferences in order to utilize interdisciplinary expertise and facilitate individual treatment plans for clients.
  • Participate in behavioral change interventions and contingency management programs
  • Refer patients as necessary to other mental health care providers and community resources/services.
  • Participate in records review to ensure that services are clinically appropriate and effective.
  • Maintain client records as required by program standards and funding requirements.
  • Participate in compliance quality assurance and quality improvement activities as directed.
  • Participate in training opportunities to advance skills and knowledge in behavioral and primary care.
  • Work within scope of training and experience and seek consultation when appropriate.
  • Adhere to all state and federal privacy regulations including HIPAA and 42 CFR Part 2 and to CCC policies and agreements regarding confidentiality privacy and security. Support compliance with all privacy and security requirements pursuant to community partners and outside providers patient confidentiality agreements including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department as well as to your supervisor or their designee.
  • Attend all mandatory CCC trainings in a timely manner.
  • Perform other duties as assigned.

Skills & Abilities:

  • Ability to consider the impacts and outcomes for underserved communities during decision-making process.
  • Ability to consider impacts of oppression structural racism and individual bias on client outcomes.
  • Knowledge evaluation and treatment planning for clients with substance use disorders including both pharmacotherapies and psychotherapies.
  • Knowledge of evaluation plans and treatment of clients with a variety of psychiatric disorders with psychopharmacological and psychotherapeutic interventions.
  • Skills in providing patient care including rapidly building rapport.
  • Ability to make treatment decisions based on assessment of patients concerns within PMHNP scope of practice.
  • Ability to provide nonjudgmental and compassionate care to vulnerable complex populations and to seek supervision as necessary
  • Ability to practice independently and seek consultation when appropriate.
  • Ability to delegate patient care as needed and if appropriate.
  • Ability to work in a busy clinic setting and responsively attend to unpredictable requests for assistance from colleagues.
  • Ability to successfully interact with a diverse population including both patients and co-workers.
  • Sufficient manual dexterity and physical ability to perform assigned tasks.
  • Ability to maintain accurate records and necessary paperwork
  • Ability to remain calm and composed when dealing with clients who are in crisis.
  • Ability to maintain strict confidentiality of patient information.
  • Excellent communication skills including diplomacy tact effective bedside manner
