Job Summary
Title - LTSS Service Care Manager
Duration - 3 months
Shift - Mon to Fri 8 am to 5 pm
Location - Austin TX
PR: $26-30.00/HR
Walk me through the day to day responsibilities of this the role and a description of the project:
- SC will have 3 days of visits and 2 days of documentation weekly
- SC will have a caseload and work assigned tasks
- SC will complete H2060 assessment
- SC will fill put required forms for each assessment
- SC will make follow up calls
What are performance expectations/metrics
- SC will complete documentation within 3 day turnaround time
- SC will stay organized and ensure work they do not fall behind
- SC will maintain a 92% or high in Audit scores.
Education Required: Bachelors degree
Licensure Required: Drivers license is required.
Years of experience required:
- 2 years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions.
Disqualifiers:
- Muat have the 2 years to the candidate will not be considered.
- No or limited experience working with people with disabilities and vulnerable populations.
Additional qualities to look for: Home Health/ nursing experience
