Duties and Responsibilities include but not limited to the following:
- Visit patientand assess patient care and plans with patient/family and physician.
- Assist the patient with activities of daily living and facilitate their efforts towards self-sufficiency and independence
- Teaches and supervises patient and family in the home environment regarding treatment self-care coping and preventive health measures.
- Communicates effectively with patient patients family agency staff physician and other involved disciplines (i.e. social services therapists etc.) to carry out treatment plan objectives.
Requirements:
- Must have a current drivers license
- Ability to travel to case assignments as assigned.
- Ability to work independently with minimal supervision and manage time effectively
- Strong interpersonal skills.
- Current license as a Speech Therapist in Pennsylvania
