Job Summary
Position Title: Licensed Practical Nurse (LPN) Primary Care
Location: Columbia PA 17512
Type: Contract
Weekly Hours: 40 hours
Schedule: 2nd Shift Monday Friday 3:30 PM 11:30 PM
Position Overview
Navitas Healthcare LLC is seeking a dedicated Licensed Practical Nurse (LPN) to provide clinical and administrative support in a primary care setting. This role ensures smooth patient flow high-quality care and effective collaboration with the healthcare team. The LPN will contribute to advancing population health management supporting behavior change and improving overall patient health outcomes.
Responsibilities
- Provide patient-centered care including taking and recording vital signs rooming patients and preparing them for exams or procedures.
- Assist providers during exams and procedures documenting visit notes in the electronic health record (EHR).
- Administer medications and vaccines including flu shots and biometric screenings.
- Respond to patient phone calls following organizational protocols.
- Educate patients on treatments care routines and health management.
- Maintain medical supply inventory stock exam rooms and provide clerical support as needed.
- Collaborate effectively with clinical and administrative staff to ensure seamless patient care.
Required Qualifications
- Recent phlebotomy experience is mandatory.
- Ability to administer flu shots and conduct biometric screenings.
- Strong attention to detail communication skills and computer literacy.
- Critical thinking decisive judgment and ability to work independently.
Education
- Completion of an accredited Licensed Practical Nurse program.
Licensure / Certification
- Active LPN license in Pennsylvania.
- Basic Life Support (BLS) certification.
Experience & Requirements
- Less than 2 years of clinical experience acceptable; recent hands-on experience is required.
- Prior experience in primary care or an outpatient setting is preferred.
