Community Living Centers Inc. provides support to individuals that have developmental disabilities in a residential environment. Our goal is to provide safe and comfortable homes that are welcomed in their communities.

We actively promote wellness of body mind and spirit by providing an enriching environment of opportunities for all of our residents.

Currently we have Full-Time Part Time and Sleep Shift positions in our community living support program with varying hours. We have 8hr shifts and 10hr and 12hr shifts based on the needs of the client. We make every effort to work around your schedule.

GENERAL FUNCTIONS: This position description is intended to be a general statement about this job and not to be considered as a detailed assignment.

SERVICE FUNCTIONS:

  1. Maintain close contact with the resident regarding medical needs; is thoroughly knowledgeable of all medical conditions medications and maintains contact with all physicians dentists psychiatrists etc.
  2. Attend Person Centered Planning meetings and provides information regarding the clients health status.
  3. Communicate with staff and outside agencies regarding the clients health status.
  4. Provide hands-on care and assistance to individuals we serve including but not limited to mobility/lifting/ transferring bathing personal hygiene and grooming laundry meal preparation and mealtime assistance.
  5. Assist individuals we serve with activities in daily living including but not limited to household tasks communication skills and activities in the community.
  6. Assist in development of individuals social and recreational interests and skills through appropriate modeling and supportive activities.
  7. Pass medications in accordance with DOCEBO training and physicians orders.
  8. Cooperate with other Client staff to assist in meeting the needs of Client in the home.
  9. Transport individual when needed however if client vehicle is not available it is expected that transportation of the resident to an appointment will occur in staffs personal car. Assure basic cleanliness.
  10. Responding to emergency situations and maintaining timely communication with the Client Doctors/Nurses Director and Family.
  11. Will accurately and efficiently complete all assigned paperwork and maintain all residents medical files in a timely manner.
  12. ADL cooking cleaning laundry and basic living standards.
  13. Other duties as assigned.

ADMINISTRATIVE FUNCTIONS:

  1. Maintain current knowledge of Employee Handbook.
  2. Perform all assignments in accordance with established policies and procedures and as instructed by supervisor.
  3. Make regular reports to immediate supervisor concerning assignments.
  4. Participate in cooperative working relationships.

RECIPIENT/STAFF RIGHT FUNCTIONS:

  1. Maintain current knowledge of and act in accordance with relevant recipient right guidelines.
  2. Maintain confidentiality of recipient/staff information.
  3. Complete all Incident Reports by the end of shift and fax them to the appropriate agencies.
  4. Report any recipient rights complaints and problems to the Office of Recipients Rights and then to your supervisor

HEALTH AND SAFETY FUNCTIONS:

  1. Follow established safety fire prevention sanitation and smoking regulations.
  2. Follow universal precautions and prevention and containment of communicable diseases.
  3. Follow and enforce proper lifting procedures.
  4. Use all equipment and supplies in a safe and efficient manner.

QUALITY ASSURANCE FUNCTIONS:

  1. Assist in meeting all established quality assurance standards.
  2. Make recommendations to enhance quality of services provided.

JOB REQUIREMENTS:

EDUCATION:

High School Diploma or GED
2 years Caregiver experience in a Private or Group Home setting.
2 years Mental Health experience.
Completion of all required CMH certifications and training for the counties of Oakland Macomb and Wayne in the State of Michigan.
Able to perform all DSP/DCW duties as needed.
Must maintain a valid and current drivers license.

ESSENTIAL FUNCTIONS

SKILLS: Ability to read and write English and perform basic mathematical calculations. Ability to comprehend and carry out written and verbal instructions. Ability to document accurately and completely. Willingness and ability to work with vulnerable adults. Have a drivers license and a good driving record. Maintain insurance on their personal vehicles.

PHYSICAL AND MENTAL REQUIREMENTS: Safely lifts and transfers person served weighing average of 80 pounds on a frequent basis. Pushes a person people in wheelchair weighing up to 80 pounds on a frequent basis often for long distances. Frequently stoops twists bends squats and kneels. Lifts overhead. Frequently lifts supplies weighing 80 pounds. Frequently pushes/pulls medication and treatment carts feels pulse rate and hears and reads blood pressure. Understands and follows instructions. Needs to see and hear the client to avoid unusual occurrences in living environment of the person served. Drives client vehicle with client as passenger; when the agency vehicle is not available the medical coordinator will need to use their own vehicle. Negative TB Test. Satisfactory physical examination.

OTHER: Ability to work within the Mission of the agency. Satisfactory verification of criminal/abuse and Sex Offenders Registry checks. Other applicable background checks as required. Reliable ongoing personal transportation.
