Licensed Mental Health Counselor

SUMMARYProvides outreach or community based outpatient mental health and/or substance abuse services to individuals groups families and couples committed to supporting those we serve reach their individualized wellness goals. Employs a range of techniques based on experiential relationship building dialogue communication and behavior change that is designed to improve the mental health of the person served or to improve group relationships (such as in a family).ESSENTIAL JOB FUNCTIONSTo perform this job successfully an individual must be able to satisfactorily perform each essential function listed below:Billable FunctionsConducts psychosocial assessments to document history and identify preliminary issues for treatment by interviewing the person served (if applicable) and transcribing or dictating relevant information for person served individual couple family and/or group therapies and family supports and crisis interventions using various treatment modalities as outlined in state/local contract and/or state licensing for providing outreach or community based outpatient mental health and/or substance abuse services to individuals groups families and mental health consultation to stakeholders and treatment team or May not be billable based on the contractDevelops individualized treatment plans in coordination with interdisciplinary team to include but may not be limited to: The Department of Children and Families District Family Safety/Community Based Care Provider Juvenile Justice Counselor (if applicable) and child targeted case manager (if one is assigned)Works with members of the interdisciplinary team to coordinate other treatment initiativesDevelops and coordinates discharge plan based on treatment indicating all relevant aftercare needs and plans. Accesses community resources that would be beneficial to persons aftercare treatment team staffing and level of care meetings as educational groups for persons served families and foster parents. Provides documentation educational in-services where applicable assuring that the skills needed for treatment plan in the home environment are and trains the family to resume care of their child (if applicable to the population being served).Conducts regularly scheduled face-to-face meetings with the person served and caretaker (if applicable) in order to monitor progress and discuss treatment strategies and -Billable FunctionsMaintains current legible and adequate medical record documentation in chart of the person being served. Assures that all required documentation is in their chart including any outside agency documentation pertaining to the person regular staff meetings clinical meetings training and one-on-one sessions with direct supervisor to process current status of court hearings as directed by other related duties and activities as RESPONSIBILITIESNone requiredMinimum Knowledge and Skills required by the JobThe requirements listed below are representative of the knowledge skill and/or abilities required to perform the job:Education and Experience:Masters Degree in health human services fieldExperience preferred or as required by stateoMassachusetts: Must have a Masters degree in social work counseling or clinical psychology or closely related field. Must have completed a full-time clinical internship in a mental health setting. License eligible preferred. Social Workers must be in process of applying for license as Certified Social Worker (LCSW).oConnecticut: Must have a Masters degree in social work counseling or clinical psychology or closely related field. Must have completed a full-time clinical internship in a mental health setting. License eligible preferredoOhio: Must hold a masters degree in social work or counseling from accredited educational institution. Must hold a license in: Licensed Social Work (LSW) Licensed Practicing Counselor (LPC) or Marriage and Family Therapist (MFT) requiredoAlabama: A masters degree in a human service field AND 2 years of post-masters experience in family and childrens services and professional licensure: Associate Licensed Counselor (ALC) Licensed Practicing Counselor (LPC) Licensed Masters of Social Work (LMSW) Licensed Clinical Social Worker (LCSW)orA masters degree in a human service field AND 5 years of post-masters experience in family and childrens servicesCertificates Licenses and Registrations:Licensed as required by drivers license and acceptable driving recordOther Skills and Abilities:N/AOther Requirements:N/APhysical Requirements:Light work.
