Join UChicago Medicineas a Transplant Nurse (RN) in our Heart and Vascular Clinic here at our main campus in Hyde Park IL. As a Pre-Transplant Coordinator you will educate patients about their upcoming evaluation and potential surgery. Monitors their pre-surgery tests lab work appointments and instructions. This position is a 100% onsite opportunity. You will need to be based in the greater Chicagoland area.

As a Transplant Nurse (RN)you will provide and coordinate the testing required for heart transplant patients in order to list them for transplant and participate in the multidisciplinary patient care services team.

In this role the Pre-Transplant Nurse is vital in the listing process. You will gather all testing results schedule needed testing and continue to update their testing while waiting to be listed or on the transplant list. You will play a major role in listing the patient in UNOS and presenting the patient to the transplant team. Essential Job Functions

  • Provides patient education and pertinent transplant information to pre-transplant recipients.
  • Monitors patient progress and answers questions related to their pre-operative course.
  • Provide patient education to patients and families on the transplant process.
  • Obtains pre-transplant consent from patients.
  • Works directly with the multidisciplinary care team and coordinates pre-transplant care of patients on the transplant list.
  • Coordinates scheduling of all pre-hospitalization medical procedures diagnostic testing clinic visits and medicine changes deemed necessary by physician /provider and/or protocol.
  • Communicates with physicians concerning patient pre-treatment regimens and related issues.
  • Coordinates data collection for pre-transplant recipients. Completes the United Network for Organ Sharing (UNOS) forms required pre-transplant.
  • Participates in patient-related consultations with physician staff as requested including mandatory attendance at multidisciplinary meetings/rounds.

Required Qualifications

  • Associate Degree in Nursing
    • Associate Degree Nurses are required to enroll in a BSN program within six (6) to nine (9) months of hire and make progress toward and complete their BSN degree within 30 months. ADN nurses receive 100% tuition reimbursement towards a BSN degree from an accredited program
  • Current licensure as a Registered Nurse in the State of Illinois
  • Must possess excellent decision-making and communication skills
  • Must possess effective teaching skills
  • Must possess basic computer skills
  • Must be familiar with insurance coverage and managed care contracts as regards coverage for transplant patients and donors if applicable

Preferred Qualifications

  • BSN degree or higher
  • Must be organized with excellent communication skills
  • Preferred candidates will have two years experience working with transplant or cardiology patients
  • A combination of transplant and other related nursing experience may be considered
  • Must be able to work within a team.

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
