Summary of Duties:
Provision of Social Work services to clients and families in Odettes Patient & Family Support Program caring for patients with gastrointestinal cancer and haematology cancer at an advanced level of practice within an interprofessional model.This includes: completion of psychosocial assessments; leading or participating in Advance Care Planning and/or Goals of Care conversations; provision of counselling and emotional support; conducting group work; locating and arranging resources; consultation/collaboration; teaching; participating in research; administration and professional development.
Qualifications/Skills:
- Masters Degree in Social Work required.
- Must be a current member in good standing with the OCSWSSW.
- Minimum 5 years previous experience working in a health care setting preferred.
- Demonstrated Psychosocial Oncology knowledge preferred.
- Psychosocial provision of care to those with gastrointestinal haematology cancer preferred.
- Proven excellent assessment counselling and problem solving skills essential.
- Superior communication negotiation mediation and collaborative interprofessional team skills required.
- Well-established knowledge of relevant health care system processes and community resources required.
- Superior time management organization and prioritization skills in addressing multiple tasks and timelines in a fast-paced environment required.
- Proven commitment to person and family-centred care essential.
- Demonstrated strong commitment to academic practice required.
- Student education required
