The Clinic RN provides comprehensive clinical nursing care including assessments administering medications and treatments per provider orders triage patient communications and deliver interventions aligned with individualized care plans. This role will collaborate with providers and the interdisciplinary team to develop implement and review participant-centered care plans. This position also coordinates participant care by communicating changes in condition supporting smooth transitions post-visit and educating participants families and caregivers on chronic disease management. The clinic RN will review contracted and internal provider documentation to reconcile orders medication changes and follow-up needs. Education Graduated from an accredited school of nursing is required. Bachelor s degree in nursing is preferred. Experience Two to three years of recent hospital nursing or other acute setting; home health and/or clinic experience preferred. Ability to set priorities manage competing demands and consistently follow through. Strong interpersonal verbal and written communication skills. Ability to communicate clearly effectively and respectfully in a wide range of situations including highly emotional or stressful interactions. Proficiency with electronic health record (EHR) systems and Microsoft Office applications. Ability to work from the office while performing telephone-based triage duties. Licenses and Certifications Current and valid Registered Nurse (RN) license in the state of Colorado maintained throughout employment.
