About the Opportunity
Star Nursing is seeking an experienced Care Management Manager Social Work to oversee hospital-based care coordination and case management services.
This leadership role manages care management operations including social work services discharge planning utilization management and patient care coordination. The Manager works closely with physicians clinical teams and community resources to ensure efficient care transitions and high-quality patient outcomes.
Position Summary
The Care Management Manager provides leadership and oversight for the Care Management Department ensuring best practices in care coordination discharge planning and resource utilization. This role collaborates with interdisciplinary healthcare teams to maintain continuity of care while supporting organizational goals related to patient outcomes and operational efficiency.
Key Responsibilities
Care Management Leadership
- Manage daily operations of care coordination case management discharge planning and social work services
- Lead care management staff to ensure high-quality patient-centered care
- Implement and monitor clinical best practices and departmental standards
Operational Oversight
- Oversee utilization management and pre-admission case management activities
- Monitor departmental performance and quality metrics
- Ensure compliance with regulatory and organizational standards
Collaboration & Communication
- Serve as liaison between patients families physicians and healthcare teams
- Coordinate with community agencies skilled nursing facilities and external partners
- Support smooth care transitions and discharge planning
Team Leadership & Development
- Recruit train and supervise care management and social work staff
- Support professional development and performance improvement
- Promote a collaborative patient-centered team environment
Minimum Qualifications
- Masters Degree in Social Work (MSW) from an accredited program
- Washington Clinical Independent Social Worker (LICSW) license
- Minimum 6 years case management experience in an acute care setting
- Minimum 2 years leadership experience at Manager or Director level
- Experience working in hospital-based care coordination programs
Preferred Qualifications
- Leadership experience in large hospital environments
- Strong knowledge of utilization management and discharge planning
- Experience managing multidisciplinary care management teams
