Position Overview
Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment in Washington DC.
This field-based role focuses on delivering high-quality patient-centered rehabilitation services in home settings. The Physical Therapist will evaluate patients develop care plans and provide evidence-based interventions to improve mobility function and independence.
The therapist will work independently while collaborating with interdisciplinary teams across assigned service areas.

Facility Type: Home Health Agency
Work Setting: Field-based (patient homes)
Charting System: HomeCare HomeBase (HCHB)
Service Area: Washington DC (assigned zip codes)
Clinical Coverage Areas
Therapists will provide care for:
Adult and geriatric patients
Post-surgical recovery
Chronic conditions (COPD CHF etc.)
Mobility and functional impairments
Orthopedic rehabilitation
Key Responsibilities
Perform in-home patient evaluations and functional assessments
Develop individualized treatment plans
Deliver therapeutic exercises gait training and mobility interventions
Complete OASIS documentation accurately and timely
Manage a caseload of approximately 56 patients per day
Monitor patient progress and adjust treatment plans accordingly
Educate patients and caregivers on home exercise programs
Maintain accurate and compliant clinical documentation
Travel between assigned patient locations efficiently
Required Qualifications
Minimum 2 years of Home Health experience
Active DC or Maryland Physical Therapist License
BLS Certification
OASIS & HomeCare HomeBase (HCHB) experience required
Valid drivers license and reliable personal vehicle
Preferred Qualifications
Previous travel experience
Experience managing independent caseloads
Strong time management skills
Scheduling & Assignment Details
40 hours guaranteed per week
Monday Friday schedule
Shift: 8:00 AM 5:00 PM
No weekend or on-call requirements
May float between zip codes within service area
Important Notes
Local candidates within 60 miles accepted at a lower rate
Mileage reimbursement: $0.58/mile (first to last patient visit)
Reliable transportation required
RTO Policy:
- No RTO within first 2 weeks
- Maximum 7 days total
- Must be submitted at time of application
Candidates must be comfortable with potential no-interview submission process
Compliance Requirements
Candidates must complete all onboarding and compliance requirements including:
Active state licensure verification
BLS certification
Two supervisor references
Skills checklist and clinical review
Pre-start modules (billable up to 4 hours)
Labs and drug screening within 48 hours of contract signing
