About the opportunity
As an Occupational Therapist in the Commonwealth Home Support Program (CHSP) team youll work with older adults who are motivated to remain living safely and independently in their own homes.
Youll conduct comprehensive assessments and provide tailored recommendations to help clients reach their goals. This may include education strategy development assistive technology prescription and home modification recommendations.
No two days are the samethis is a varied and rewarding role with a strong focus on person-centred care and occupational outcomes.
Youll be part of a supportive team including Grade 3 2 and 1 Occupational Therapists and Allied Health Assistants.
The CHSP team is embedded within the broader Occupational Therapy department at Werribee Mercy Hospital offering opportunities to collaborate contribute and grow professionally across services.
We offer regular supervision via a structured support system and encouragement to pursue ongoing professional development aligned with your career goals.

To thrive in this role you will have:
A recognised degree in Occupational Therapy and AHPRA registration.
A valid and current unrestricted drivers license.
Demonstrated understanding and experience of aged care issues the aged care service system (My Aged Care) and empathy with older adults is essential.
The ability to work autonomously make decisions organise own workload and monitor own performance to achieve predetermined targets.
A compassionate flexible and positive approach to effective patient care and management.
The ability to nurture friendly respectful relationships and contribute to a collaborative team environment.
You will also be required to provide evidence of or in the process of obtaining:
A current Police Record Check and Working with Children Check
Proof of current influenza vaccination and other vaccination status consistent with public health orders.
