What Youll Do

As a Home Health Registered Nurse you will play a vital role in coordinating and delivering high-quality patient care in the home setting.

Key responsibilities include:

• Conduct comprehensive assessments of patients and families to identify physical psychosocial and environmental needs

• Develop implement and manage individualized plans of care in collaboration with physicians and the interdisciplinary team

• Perform initial evaluations and ongoing reassessments to monitor patient progress

• Provide skilled nursing care and education to patients and caregivers

• Communicate significant findings changes in condition and safety concerns with physicians clinical leadership and care team members

• Update and revise care plans based on patient needs and clinical outcomes

• Maintain accurate and timely documentation in accordance with regulatory and organizational standards

• Ensure strict compliance with HIPAA privacy regulations and patient confidentiality

• Travel within the service area to provide care in patients homes

What Were Looking For

• Current unrestricted Registered Nurse (RN) license in the state of practice

• 1 year of clinical nursing experience

• Ability and willingness to travel within the assigned coverage area

• Ability to lift push pull or support up to 50 pounds including assisting with patient positioning or transfers

• Valid drivers license reliable vehicle and current auto insurance per state requirements

• Associate or Bachelors Degree in Nursing (BSN preferred)

• Home Health or Hospice experience preferred

• Experience with HomeCare HomeBase (HCHB) preferred

• Sit stand bend lift and move intermittently and be able to lift 50-100 lbs.
