In this role youll play a vital part in coordinating medical prevention and other support services for people living with Hepatitis C and/or HIV in order to assure access to and retention in care. Essential Job Duties:
- Conduct case investigations and follow-up on reportable infectious diseases (HIV HCV STIs) in accordance with state and local guidelines.
- Perform community outreach to identify notify and engage individuals at risk or diagnosed including completing interviews and risk assessments.
- Link individuals to medical care treatment and supportive services and support retention in care through ongoing follow-up.
- Coordinate with healthcare providers public health agencies and community partners to ensure continuity of care and service delivery.
- Maintain accurate and timely documentation in disease reporting systems (e.g. ODRS) and generate required reports.
- Provide education on disease prevention harm reduction and available resources to patients staff and community partners.
- Support outbreak response efforts including contact tracing surge capacity activities and participation in public health initiatives.

Requirements
Qualifications:
- High School Diploma or its equivalent.
- Experience in social science or public health health education or patient advocacy.
- Experience working with culturally and economically diverse populations including people of color LGBTQ populations and people living with chronic disease substance users and persons with mental health diagnosis.
- Within the first 90 days of employment: BLS and completion of health screenings as outlined in the credentialing policy are required for successful completion of orientation.
- Ability to be insured on the commercial auto insurance policy based on driving record including active drivers license and personal automobile insurance and reliable transportation.
