SUMMARY: Performs a variety of tasks as an integral part of the client support team requiring collaborative work with teams strong communication skills and the ability to form long-lasting supportive relationships. Provides referral and resources for client needs and provides wrap around services for clients.
ESSENTIAL DUTIES AND RESPONSIBILITIES: Include the following. Other duties may be assigned.
Assess clients needs.
Assist client with developing a plan.
Monitor client progress.
Establish and maintain positive relationships with clients.
Refer clients to appropriate community resources.
Coordinate necessary wrap-around services in the community.
Complete the necessary administrative tasks.
Consistently demonstrate professionalism with communication with peers and patients.
Develop and maintain strong positive relationships with tribal and non-tribal providers and cohorts.
Support outreach events between Squaxin and all partners.
Coordinate client supportive services.
Collect and analyze data to measure the implementation and impact of the program.
Maintains confidentiality of client information issues and treatment plans.
Other duties as assigned.
SUPERVISORY RESPONSIBILITIES: This position has no supervisory responsibilities.

Able to work a flexible schedule including some weekend and frequent evening hours.
EDUCATION and/or EXPERIENCE: AA in the Human Services field & four-years of relevant experience OR High School Diploma or GED and six years of relevant experience. Experience working with Native Americans is preferred.
LANGUAGE SKILLS: Ability to read analyze and interpret general business periodicals professional journals technical procedures or governmental regulations. Ability to write reports business correspondence and procedure manuals. Ability to effectively present information and respond to questions from groups of managers clients customers and the public. Ability to engage and manage a training group toward achieving successful outcomes. Must be able to speak before groups in an engaging manner that maintains attention and work with a team to implement conferences and training.
MATHEMATICAL SKILLS: Ability to calculate figures and amounts such as discounts interest commissions proportions percentages area circumference and volume. Ability to apply concepts of basic algebra and geometry.
REASONING ABILITY: Ability to apply common sense understanding to carry out instructions furnished in written oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
CERTIFICATES LICENSES REGISTRATIONS: Must have a current valid Washington State Drivers License. The successful applicant must pass a criminal history background. Must be insured by the Tribes insurance carrier.

While performing the duties of this job the employee is regularly required to stand walk and use hands to handle or feel reach with hands and arms stoop kneel crouch or crawl and talk or hear. The employee is occasionally required to sit climb or balance and taste or smell. The employee must regularly lift up to 25 pounds frequently lift up to 50 pounds and occasionally lift more than 100 pounds. This position also requires close vision distance vision color vision peripheral vision depth perception and ability to adjust focus.

To perform these duties the position requires sitting standing moving and lifting at a minimum of 8 hours. Also requires reporting and need to update with Microsoft word and minimal excel knowledge.
OTHER: Certifications and credentials will be taken into consideration. Knowledge of industry is desirable. The job requires the ability to work independently with limited supervision as well as part of a team. Regular and consistent attendance is a condition of employment. The employee must deal courteously with the public patients and staff. Applicants who have been a patient of the NWITC programs must have completed behavioral health services for a minimum of six months and should have six months of sustained recovery.
