Empath Home Health a member of Empath Health is seeking a Home Health Intake Coordinator PRN/ Part-time. The Home Health Intake Coordinator is responsible for efficiently coordinating the referral process ensuring a seamless review response andcarecoordination for patients referred to the Organization.

Location: Work on-site at a Empath Home Health Florida branch (Naples or Fort Myers)

Schedule: Flexible schedule AM or Afternoons some Saturday or Sunday (preferred). Training in Fort Myers or Naples on-site

What Youll Do

• Serves as the primary point of contact for both internal and external customers providing exceptional customer service.
• Receives and meticulously documents patient referrals for home health services ensuring strict adherence to applicable coverage criteria.
• Reviews referrals to determine the appropriate care setting and communicate effectively with the care center Business Development and referral sources as needed.
• Gathers essential information to assist the clinical team in scheduling evaluation visits.
• Communicates pertinent patient information to the care center team through approved processes.
• Coordinates with other departments for insurance verification and obtains payer authorization as necessary.
• Professionally communicates and follows up with referral sources regarding required documentation for a comprehensive referral.
• Meets productivity expectations with quality documentation of complete referrals and all required follow-ups.

What Youll Need

• High School diploma or GED equivalent.
• Two (2) years proven experience in Home Health referral management or a related field is preferred.
• Clear understanding of medical terminology and basic ICD-10 coding.
• Proficient in Microsoft suite of products including Outlook Word and Excel.
• Reliable internet service and the ability to work from home with minimal disruption to the workday.
• Home Health experience is preferred.
