WhatYoullDo

As a Home Health Licensed Practical Nurse (LPN) you will:

• Provide home-based care to patients with illnesses injuries and chronic diseasesoften after hospital dischargeunder the supervision of an RN.
• Work with a team of physicians RNs CNAs and aides to deliver quality patient care.
• Take patient vitals do assessments document progress and report changes to RN supervisor.
• Assist RN in carrying out patients plan of care and teaching patient self-care techniques.
• Educate family on the patients plan of care medication and home safety.
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge.

What Youll Need

• Graduate of a practical (vocational) nursing program for LPN/LVN
• Current license as an LPN/LVN in the South Carolina.
• At least one-year or 1200 hours of experience as an LPN/LVN.
